Case No. 2007-001818

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $10,000.00 plus costs against Adler Interior Design Associates, Inc. and Gary Adler.  Mr. Adler continued to offer interior design services even though he previously signed an affidavit agreeing to change his business name and stop offering interior design services.

 

Jose Ismael Allendes
Case No. 2006-060092

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $15,000.00 plus costs against Jose Allendes.  Mr. Allendes’ title block on a set of residential drawings offers architectural services.  Further, Mr. Allendes is offering such services through a fictitious name without a certificate of authorization.

 

Susan Andrews
Case No. 2004-035419
 

The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $15,000.00 plus costs against Susan Andrews.  Ms. Andrews is not licensed to practice interior design in Florida and contracted to perform such services for a condominium.  After Ms. Andrews received the retainer for $5,000.00 she disappeared and never provided any of the services.

 

David Beatty
Case No. 2005-015496
 

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement imposing a fine of $500.00 fine against David Beatty. The case was based upon a citation which was issued for failure to complete the code curriculum course.  Mr. Beatty’s license will be suspended until he comes into compliance.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation placing a reprimand against Tony Benton’s Florida license to practice architecture and imposing a fine of $1,000.00 plus costs.  Mr. Benton was the successor architect for an airplane hanger and never notified the original architect in writing.  Mr. Benton also signed and sealed engineering plans that he was not competent to perform.  Further, Respondent’s letterhead does not contain his license number and his title block was in violation.

 

Dean Bolaris

Case Nos. 2005-031944 & 2005-027571
 

The Florida Board of Architecture and Interior Design agreed to enter a final order placing a reprimand and imposing a fine of $3,500.00 plus costs against Dean Bolaris license to practice architecture.  This involved two separate complaints.  The first complaint involved Mr. Bolaris and a retainer on two projects.  On one project, Mr. Bolaris did not provide anything and on the other, the drawings were unsatisfactory.  In the second complaint, it appeared that Mr. Bolaris overcharged the client and would not provide a refund.  Both cases should be civil in nature but because Mr. Bolaris did hot properly respond to the investigations, it was difficult to determine.  Further, Mr. Bolaris was operating through a business entity without a certificate of authorization.

 

Christian Crookless

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement imposing a fine of $500.00 fine against Christian Crookless. The case was based upon a citation which was issued for failure to complete the code curriculum course.  Mr. Crookless license will be suspended until he comes into compliance.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $5,000.00 plus costs against Jimmy Dean.  Mr. Dean contracted to provide signed and sealed drawings for a commercial hair salon.  Further, Mr. Dean is offering such services without a certificate of authorization.

 

Judith Ellis

Case No. 2006-056940
 

The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $1,000.00 plus costs against Judith Ellis.  Ms. Ellis is not licensed to practice interior design in Florida but made a proposal to provide design services for the common area in a condominium project.

 

Raymond Lee Enfield

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation placing a reprimand against Raymond Enfield’s license to practice of architecture and imposing a fine of $2,000.00 plus costs.  Mr. Enfield is licensed in Florida but had his Tennessee license revoked.  The Tennessee Board found that Mr. Enfield failed to exercise proper responsible supervisory control when he signed and sealed a project manual for the design of a modular building unit.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $2,500.00 plus costs against Daniel Estevill.  Mr. Estevill is not licensed to practice architecture in Florida and contracted to provide such services for a residence.  Further, Mr. Estevill is offering such services through a business entity without a certificate of authorization.

The Florida Board of Architecture and Interior Design agreed to enter a final order  imposing a fine of $5,000.00 plus costs against Michael Fletcher.  Mr. Fletcher is not licensed to practice architecture in Florida and held himself out as an architect to a client for a residential project.

 

Donald Scott Fraser

Case No. 2007-010309
 

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation placing a reprimand and imposing a fine of $1,500.00 against Donald Fraser’s license to practice architecture.  An unlicensed individual entered into a contract to perform design services for a commercial project.  The unlicensed individual prepared drawings for the project that were signed and sealed by the Subject.  The Subject did not maintain the proper documentation for signing plans prepared outside of his office.  Further, Mr. Fraser was offering architectural services through a business entity without a certificate of authorization.

The Florida Board of Architecture and Interior Design agreed to enter a final order revoking Sharon Gold’s license to practice architecture in Florida.  Ms. Gold failed to satisfy the terms of a final order that was previously entered by failing to pay a $500.00 fine.

 

Fernando Gomez-Pina
Case No. 2006-015015
 

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $2,500.00 plus costs against Fernando Gomez-Pina.  Mr. Pina is a licensed professional engineer but not a licensed architect in Florida and signed and sealed plans for a residential renovation.  The plans were prepared by an unlicensed draftsman that entered into a contract with the client to perform architectural services.

 

Halifax Services, Inc.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $2,500.00 plus costs against Halifax Services, Inc. and Mr. Kaprocki.  Halifax Services, Inc. provided signed and sealed drawings for a residential addition and porch remodeling.  Mr. Kaprocki was offering such services through a corporation without a certificate of authorization.

 

Mark A. Hunt
Case No. 2007-000489
 

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $5,000.00 plus costs against Mark Hunt.  Mr. Hunt was previously licensed; however, his license went null and void on March 1, 1999.  Mr. Hunt attempted to reapply for licensure in 2004 but never followed through.  On April 6, 2006, Mr. Hunt entered into a contract to provide consulting services for a commercial project.  The contract held Mr. Hunt as out as an architect and contains his null and void license number.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $2,500.00 plus costs against Marc Janecki.  Mr. Janecki is not licensed to practice interior design or architecture in Florida and contracted to provide interior design services on a condominium project.  Mr. Janecki held himself out as an interior designer to the client and the contract offered architectural details.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $2,500.00 plus costs against JRG Incorporated.  The firm is offering architectural services on its title block.

 

John Lamb
Case No. 2007-008620
 

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation placing a reprimand and imposing a fine of $3,000.00 against John Lamb’s Florida license to practice architecture.  Mr. Lamb signed and sealed plans for a residential project that were prepared by an unlicensed draftsman without ever meeting with the client.  Further, the unlicensed draftsman entered into the contract with the client to provide architectural services.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $2,500.00 plus costs against Paul Li.  Mr. Li is a licensed professional engineer but not a licensed architect in Florida and signed and sealed plans for a commercial project without the involvement of an architect.  The architectural drawings were prepared by an unlicensed draftsman that entered into a contract with the client and held himself out as an architect.

The Florida Board of Architecture and Interior Design agreed to enter a final order  imposing a fine of $10,000.00 plus costs against Doug McIntee.  Mr. McIntee is not licensed to practice architecture in Florida and entered into a contract to provide architectural services for a commercial property.  Further, the letterhead on the contract offered architectural services.

 

William Michaelski, Jr.

The Florida Board of Architecture and Interior Design agreed to enter a final order  imposing a fine of $15,000.00 plus costs against William Michaelski.  Mr. Michaelski entered into an oral contract to perform architectural services for a retail office shop.  Mr. Michaelski held himself out as an architect to the Complainant.  Further, Mr. Michaelski was paid but never produced any drawings.

The Florida Board of Architecture and Interior Design agreed to enter a final order  imposing a fine of $15,000.00 plus costs against Earl Nelson.  Mr. Nelson was previously licensed to practice architecture in Florida; however, his license went null and void on March 1, 1995.  The complaint was based on a case in which Mr. Nelson practiced architecture, held himself out as an architect and used his null and void license number.

The Florida Board of Architecture and Interior Design agreed to enter a final order  imposing a fine of $10,000.00 plus costs against David Pillsbury.  Neither the firm nor Mr. Pillsbury are licensed to practice architecture or interior design in Florida and was offering interior design services under the interior design section of the Ocala Yellow pages.  Mr. Pillsbury failed to respond to this investigation.  Further, Mr. Pillsbury was previously disciplined for practicing AR without a license.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $2,250.00 plus costs against Pamela Niemann.  Ms. Niemann is not licensed to practice interior design in Florida and contracted to provide such services on a commercial project.  Further, Ms. Niemann is offering such services through a business entity without a certificate of authorization.

The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $500.00 fine against Sandra Ortiz. The case was based upon a citation which was issued for failure to complete the code curriculum course.  Ms. Ortiz’ license will be suspended until she comes into compliance.

The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $500.00 fine against Sue Ramsey. The case was based upon a citation which was issued for failure to complete the code curriculum course.  Ms. Ramsey’s license will be suspended until she comes into compliance.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $5,000.00 plus costs against Residential Architectural Design & Drafting, Inc.  The firm provided architectural services to convert a residence to a commercial building.  Mr. Paultre is doing business as Residential Architectural Design & Drafting, Inc.  Further, Mr. Paultre is offering such services through a business entity without a certificate of authorization.

The Florida Board of Architecture and Interior Design agreed to enter a final order  imposing a fine of $10,000.00 plus costs against Mark Robinson.  Mr. Robinson is not licensed to practice architecture in Florida and entered into a contract to provide signed and sealed drawings for a residential project.

The Florida Board of Architecture and Interior Design agreed to enter a final order  imposing a fine of $15,000.00 plus costs against Doug Spencer.  Mr. Spencer is not licensed to practice architecture in Florida and prepared a proposal to provide architectural services for a church.  Further, Mr. Spencer performed these services through a business entity without a certificate of authorization.

 

Roland Stout

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation placing a reprimand against Roland Stout’s license to practice architecture in Florida and imposing costs.  Mr. Stout signed and sealed engineering drawings copied from the Cameron/Ashley Building Product, aluminum construction grade without their permission.  The Board’s expert, Jerry Hicks, reviewed the file and concurred with the violation.

 

James E. Taylor

The Florida Board of Architecture and Interior Design agreed to enter a final order  imposing a fine of $15,000.00 plus costs against James Taylor.  Mr. Taylor is not licensed to practice architecture in Florida and prepared drawings to remodel a residence.  Mr. Taylor’s business card, letterhead and title block offer architectural services.  Further, Mr. Taylor held himself out as an architect to the client.

The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $500.00 fine against Marilyn Tully. The case was based upon a citation which was issued for failure to complete the code curriculum course.  Ms. Tully’s license will be suspended until she comes into compliance.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $4,000.00 plus costs against Debra Yates.  Ms. Yates is not licensed to practice interior design in Florida and was offering such services in an advertisement.  Further, Ms. Yates’ website was offering architectural services and provided pictures of commercial projects.
