Sam Carter Case No. 2003-079935
The Florida Board of Architecture and Interior Design agreed to
enter a final order approving a settlement stipulation imposing a
$1,000.00 fine and costs of $282.00 against Sam Carter. The
complaint alleged that Mr. Carter contracted with a building supply
company in Live Oak, Florida to provide architectural services for
the renovation of a commercial building and prepare architectural
drawings for the project. Mr. Carter is not a licensed architect in
the State of Florida.
Brent Cornwell Case No. 2003-076953
The Florida Board of Architecture and Interior Design agreed to
enter a final order revoking Mr. Cornwell's license to practice
interior design. While Respondent was licensed, a final order was
entered on February 28, 1997, requiring Respondent to pay a
$1,250.00 fine within thirty days. Respondent failed to comply with
the terms of the final order.
John Eisenlau Case No. 2003-070802
The Florida Board of Architecture and Interior Design agreed to
enter a final order approving a settlement stipulation imposing a
$5,000.00 fine and costs of $198.00 against John Eisenlau. The
complaint alleged that Mr. Eisenlau is licensed in Georgia but not
in Florida and he offered to provide architectural services on the
Marion County Jail expansion project and held himself out as an
architect.
Robert Hermann Case No. 2002-11607
The Florida Board of Architecture and Interior Design agreed to
enter a final order approving a settlement stipulation placing
Robert Hermann's license to practice architecture on suspension for
a period of six months followed by three years of probation and
imposing a $5,000.00 fine and costs of $342.00. The complaint
alleged that Mr. Herrmann was the architect of record an office
complex in Bonita Springs, Florida. J. Kosinksi, was the structural
engineer of record for the project under an agreement with the
Respondent. Mr. Kosinksi signed and sealed the drawings and
submitted them for permit. The Building Department requested changes
to the drawings. Mr. Hermann revised the structural drawings that
were prepared by Kosinski and resubmitted them to the Building
Department. Mr. Kosinksi notified the Building Department that the
revised drawings contained structural modifications that were not
reviewed by him and it contained modifications that not were not
authorized or incorporated by him and he objected to their use. Mr.
Hermann did not supervise the preparation of the plans and drawings
he revised.
Michael J. Ingram Case No. 2003-004427
The Florida Board of Architecture and Interior Design agreed to
enter a final order revoking Mr. Ingram's license to practice
interior design and imposing a $5,000.00 fine and costs against Mr.
Ingram. Mr. Ingram was previously a licensed interior designer in
the State of Florida. While Mr. Ingram was licensed a final order
was entered requiring him to pay a fine. Mr. Ingram failed to comply
with the terms of the final order. Mr. Ingram responded and he does
not intend to pay; however, he did not dispute the allegations in
the administrative complaint.
Harold E. Langbehn Case No. 2003-093637
The Florida Board of Architecture and Interior Design agreed to
enter a final order suspending Harold Langbehn's license to practice
architecture for six months and imposed a fine of $750.00 and costs
of $270.00. Mr. Langbehn's license to practice architecture in South
Carolina was suspended for six months. The South Carolina Board
found that Mr. Langbehn's design drawings for a church displayed a
lack of competence and technical skill.
Roberto Lence Case No. 2003-061697
The Florida Board of Architecture and Interior Design agreed to
enter a final order approving a settlement stipulation placing
Roberto Lence's license to practice architecture on probation for a
period of two years and imposing a $5,000.00 fine and costs of
$396.00. The complaint alleged that Mr. Lence signed and sealed two
commercial projects that were prepared by an unlicensed draftsman
without complying with all of the requirements of exercising proper
responsible supervisory control for plans prepared outside of an
architect's office.
Stephanie B. Ollie Case No. 2003-004496
The Florida Board of Architecture and Interior Design agreed to
enter a final order imposing a $1,885.00 fine and costs against Ms.
Ollie. Ms. Ollie was previously a licensed interior designer in the
State of Florida. While she was licensed, a final order was entered
requiring her to pay a $1,000.00 fine within thirty days. Ms. Ollie
failed to comply with the terms of that final order.
Antonio Pagan Case No. 2003-077407
The Florida Board of Architecture and Interior Design agreed to
enter a final order imposing a $5,000.00 fine and costs of $258.00
against Antonio Pagan. The complaint alleged that Mr. Pagan prepared
a proposal to design plans for a church. Mr. Pagan was also offering
construction documents for permitting and commercial design services
over the Internet. Mr. Pagan is not licensed to practice
architecture in the State of Florida.
Juan Rodriguez-Tellaheche Case No. 2002-007631
The Florida Board of Architecture and Interior Design agreed to
enter a final order approving a settlement stipulation imposing a
$5,000.00 fine and costs of $126.00 against Juan
Rodriguez-Tellaheche. The complaint alleged that Mr.
Rodriguez-Tellaheche entered into a contract to perform
architectural services for a property and held himself out as an
architect. He was previously a licensed architect but his license
was placed in a null and void status for failing to renew.
Dan F. Voelker Gulf Coast Architectural and Building Case
No. 2003-042587
The Florida Board of Architecture and Interior Design agreed to
enter a final order imposing a fine of $15,000.00 and costs of
$294.00 against Mr. Voelker. The complaint alleged that Mr. Voelker
offered architectural services to the public on a business card
using a license number of a company whose license is now null and
void for failing to renew. Mr. Voelker is not licensed to practice
architecture in the State of Florida.
