The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $5,000.00 plus costs against Henri Almanzar and Concepts & Designs. Mr. Almanzar is not licensed to practice interior design or architecture and contracted to provide architectural services for a residential project. Further, Mr. Almanzar is holding himself out as an architect and interior designer and is offering such services through a corporation without a certificate of authorization.

Ms. Carruthers is not licensed to practice architecture and entered into a contract to provide architectural services for a residential project. Further, Ms. Carruthers is offering such services through a corporation without a certificate of authorization.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $500.00 plus costs and completing 18 hours of continuing education against William M. Leveille’s license. Mr. Leveille is a licensed architect and affirmed at renewal that he had completed the required continuing education hours. Mr. Leveille was chosen for an audit of the continuing education required and has provided proof of only 12 hours during the audit period.

Ms. Meehan was previously licensed to practice interior design however, her license went delinquent on March 1, 2003 and then into a null and void status on March 1, 2005. Ms. Meehan continues to offer interior design services and use a null and void license. Further, Ms. Meehan’s web site offers “architectural working drawings”.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $3,250.00 plus costs against Montero Design, Inc. and Tony Montero. Mr. Montero is not licensed to practice interior design and contracted to provide interior design services on two commercial projects. Further, Mr. Montero is offering such services through a business entity without a certificate of authorization.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $750.00 plus costs and completing 27.5 hours of continuing education against Phillip D. Morgan’s license. Mr. Morgan is a licensed architect and affirmed at renewal that he had completed the required continuing education hours. Mr. Morgan was chosen for an audit of the continuing education required and has provided proof of only 7.5 hours during the audit period.

The Florida Board of Architecture and Interior Design agreed to enter a final order placing a reprimand and probation until proof of continuing education against Mr. Olbekson’s license to practice architecture. Mr. Olbekson was practicing on a suspended license for failure to take the building code course.

Norman D. Palmer

The Florida Board of Architecture and Interior Design agreed to enter a final order revoking Norman D. Palmer of Gainesville, Florida license to practice architecture. Mr. Palmer signed and sealed drawings that were prepared by an unlicensed individual for a residential project while his license was in a delinquent status. Further, Mr. Palmer has not provided the required documentation that he exercised proper responsible supervisory control for plans prepared outside of his office.

Dale H. Parsons

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $1,000.00 plus costs and completing 40 hours of continuing education and suspension until compliance against Dale H. Parsons’ license. Mr. Parson is a licensed architect and affirmed at renewal that he had completed the required continuing education hours. Mr. Parsons was chosen for an audit of the continuing education required and has not provided proof of any hours during the audit period.

Mr. Prato is a licensed architect and affirmed at renewal that he had completed the required continuing education hours. Mr. Prato was chosen for an audit of the continuing education required and has provided proof of only 15 hours during the audit period.

James Michael Rinehart

The Florida Board of Architecture and Interior Design agreed to enter a final order reprimanding James Michael Rinehart’s license to practice interior design, placing his license on probation for a period of two years and imposing a fine of $1,000.00 plus costs. Mr. Rinehart failed to produce permit and construction documents after receiving $15,000.00. This case was reviewed by Emory Johnson who found violations.

Mr. Sable is a licensed architect and affirmed at renewal that he had completed the required continuing education hours. Mr. Sable was chosen for an audit of the continuing education required and has provided proof of only 15 hours during the audit period.

The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $1,000.00 plus costs against Jerry Thibodeau and Thibodeau Builders Corp. Mr. Thibodeau is not licensed to practice architecture and prepared architectural drawings for a residential and signed and sealed the drawings using his corporate seal. Mr. Thibodeau’s title block holds him out as an architect. Further, Mr. Thibodeau is offering such services through a corporation without a certificate of authorization.

David R. Tichenor

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation placing a reprimand and imposing a fine of $3,000.00 plus costs against David R. Tichenor’s license. Mr. Tichenor is a licensed architect and signed and sealed plans for a commercial project that were prepared by an unlicensed draftsman without ever meeting with the client. Further, Mr. Tichenor has been unable to provide the proper documentation for signing and sealing drawings prepared outside of his office.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation placing a reprimand and imposing a fine of $5,000.00 plus costs and two years probation against Angel Torres-Collazo’s license. Mr. Torres-Collazo is a licensed architect and signed and sealed drawings that were prepared by an unlicensed draftsman without ever meeting with the client. Further, the plans appear to lack sufficient detail and constituted negligence in the practice of architecture.

Wills’ license. Mr. Willis is a licensed architect and affirmed at renewal that he had completed the required continuing education hours. Mr. Willis was chosen for an audit of the continuing education required and has not provided proof of any hours during the audit period.
