Further, Ms. Duke is offering such services through a business entity without a certificate of authorization.

An unlicensed individual entered into a contract to provide architectural drawings for a residential project. Mr. Giwa acted as the architect for the project without ever meeting with the client.

It has been over a year and a half and the web site has not been changed. Further, in a recent advertisement, Ms. Flack is offering commercial design services.

Further, Mr. Thompson is offering these services through a business entity without a certificate of authorization.

Ms. Irizarry does not hold herself out as an interior designer, however, the contract is to perform design services for a doctor’s office. Ms. Irizarry also has an advertisement in which she offers “architectural interiors”. Further, Ms. Irizarry is offering these services through a business entity without a certificate of authorization.

Dennis Mitchell

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation placing a reprimand and imposing a fine of $3,000.00 plus costs against Dennis Mitchell. Mr. Mitchell is licensed to practice architecture, however, Mr. Mitchell signed and sealed drawings that were prepared by an interior design firm. Mr. Mitchell has been unable to provide documentation that he properly provided responsible supervisory control for documents prepared out of his office. Mr. Mitchell was also disciplined by the Nevada Board for failing to exercise proper responsible supervisory control for a project in that state.

The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $5,000.00 plus costs against Jamal S. Nagamia. Mr. Nagamia is a licensed Professional Engineer. Mr. Nagamia signed and sealed plans for a church without the involvement of an architect. The architectural drawings were prepared by an unlicensed draftsman that entered into a contract with the client that offered architectural services.

The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $15,000.00 plus costs against Earl G. Nelson. Mr. Nelson is not licensed to practice architecture. Mr. Nelson was previously licensed; however, his license went delinquent on March 1, 1993, and then into a null and void status on March 1, 1995. The complaint is based on a case in which Mr. Nelson practiced architecture, held himself out as an architect and used his null and void license number.

Mr. Russell signed and sealed the drawings without maintaining the proper documentation for plans prepared outside of his office and his title block on the plans is in violation.
