Case No. 2005-037381
Probable Cause was found that Johanna Afanador of Miami, Florida is not
licensed to practice interior design in the State of Florida and held herself
out as an interior designer in a letter to a client. A Notice and Order to Cease
and Desist was issued and an Administrative Complaint seeking fines was filed.
AMD Group of South Florida, Inc.
Gina Guerrier
Case No. 2005-048846
Probable Cause was found that AMD Group of South Florida, Inc. of Plantation,
Florida is not a licensed architectural firm in the State of Florida and is
listed under the Architect heading in the yellow pages and is also offering
commercial services. A Notice and Order to Cease and Desist was issued along
with the requirement that Ms. Guerrier sign an affidavit stating the firm will
refrain from offering architectural or commercial services and will no longer
advertise in the yellow pages under the Architect section. If Ms. Guerrier fails
to return the affidavit, an Administrative Complaint seeking fines will be
filed.

Probable Cause was found that Carrie’s Interiors of Punta Gorda, Florida is
not a licensed interior design firm in the State of Florida. The firm is
offering interior design services in an advertisement that appeared in the
Charlotte County Sun Newspaper. A Notice and Order to Cease and Desist was
issued and an Administrative Complaint seeking fines was filed.

offering interior design services. Ms. Lansdown entered into four contracts for
interior design services. Three were for condominiums and the other was for a
model home. Each contract offered interior design and architectural services.
Further, Ms. Lansdown held herself out as an interior designer. A Notice and
Order to Cease and Desist was issued and an Administrative Complaint seeking
fines was filed.

interior design services on a residence. Mr. Marquette held himself out as an

Probable Case was found that Bradley Thompson of Palm Harbor, Florida is not
licensed to practice interior architecture in the State of Florida and provided
architectural services for a commercial project owned by his mother. Further,
Mr. Thompson is doing business as Architecture, Design & Construction Management
without a certificate of authorization. A Notice and Order to Cease and Desist
was issued and an Administrative Complaint seeking fines was filed.

Probable Case was found that Virtual 3D, Inc. of Boca Raton, Florida is not
licensed to practice architecture in the State of Florida and is offering
services under the architect heading in the yellow pages. A Notice and Order to
Cease and Desist was issued along with the requirement that David Karram sign an
affidavit stating that the firm will refrain from offering architectural
services. In the future, Mr. Karram will change the advertisement in the yellow
pages and will no longer advertise under the architect heading.

Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Addis when he renewed his license. Mr. Addis affirmed at renewal
that he had completed the required hours; however, the audit showed a
deficiency. An Administrative Complaint seeking discipline was filed.

Probable Cause was found that Stephen J. Alexander of Indianapolis, Indiana,
a licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Alexander when he renewed his license. Mr. Alexander affirmed at
renewal that he had completed the required hours; however, the audit showed a

Probable Cause was found that Kenneth Buyyououski of Wycoff, New Jersey a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Buyyounuoski when he renewed his license. Mr. Buyyounuoski was
chosen for the audit and has not provided any evidence for taking continuing

Probable Cause was found that Warren Lisenbee of Crestview, Florida, a

Statutes. Mr. Lisenbee signed engineering sheets for a residential project on
two projects that lacked sufficient detail to certify windloading. The plans
were reviewed by the expert, Jerry Hicks who determined that there is sufficient

Probable Cause was found that Nancy Neuberg-Wright of Stuart, Florida, a
licensed interior designer in the State of Florida, violated Chapter 481,

licensed interior designer in the State of Florida, violated Chapter 481,
Florida Statutes. Mrs. Sachs entered into a contract to perform design services
for a condominium. Mrs. Sachs is offering interior design services through a
corporation without a Certificate of Authorization. A Citation was issued.
Julio Sanchez
Case No. 2005-027046
Probable Cause was found that Julio Sanchea of Miami, Florida, a licensed
architect in the State of Florida, violated Chapter 481, Florida Statutes. Mr.
Sanchez signed and sealed drawings that were below minimum acceptable standards.
Mr. Sanchez claims the plans were never intended to be final construction
documents and were intended for zoning review. The plans were reviewed by
expert, Jerry Hicks, who determined that a violation occurred. An Administrative
Complaint seeking discipline was filed.

Probable Cause was found that F T Schmidt of Brooklyn, New York, a licensed
architect in the State of Florida, violated Chapter 481, Florida Statutes. Mr.
Schmidt failed to provide to the Board of Architecture and Interior Design proof
of completion of the core curriculum courses, or pass the equivalency test of
the Building Code Training Program. An Administrative Complaint seeking
discipline was filed.

licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. Mr. Stout signed and sealed a report that was below minimum acceptable
standards. An addition to a residence had been constructed without any permits.
The report was reviewed by expert, Jim Anstis, who determined that a violation
