Blue Sky Environments, Inc.
Case No. 2005-053233
Probable Cause was found that Blue Sky Environments, Inc. of Coral Springs,
Florida is not licensed to practice interior design in the State of Florida and
is offering commercial design services in an advertisement in the Florida Design
Magazine. A Notice and Order to Cease and Desist was issued and an
Administrative Complaint seeking fines was filed.
Geoffrey C. Chick, Jr.
Geoff Chick & Associates, Inc.
Case No. 2006-001724
Probable Case was found that Geoffrey Chick of Miramar Beach, Florida is not
licensed to practice architecture in the State of Florida and is advertising
under the architect heading in the phone book. Mr. Chick is also listed as an
architect on a job site sign and is on an approved list of architects for a
large residential project. A Notice and Order to Cease and Desist was issued
along with the requirement that Mr. Chick sign an affidavit stating he will
refrain from offering architectural services or words to that effect and will no
longer advertise under the architect section. If Mr. Chick fails to return the
affidavit, an Administrative Complaint seeking fines will be filed.
Carrie S. Christy
Imaginations
Case No. 2005-065414
Probable Cause was found that Carrie S. Christy of Tallahassee, Florida is
not a licensed interior designer in the State of Florida and is offering
commercial and interior design services in an advertisement. A Notice and Order
to Cease and Desist was issued and an Administrative Complaint seeking fines was
filed.
Clara T. Garcia
Adonai Design & Construction, Inc.
Case No. 2006-002296
Probable Cause was found that Clara T. Garcia of Miami, Florida is a licensed
general contractor but not licensed to practice architecture in the State of
Florida. Ms. Garcia told the complainant she was and architect and is offering
architectural services through a business entity without a certificate of
authorization. A Notice and Order to Cease and Desist was issued and an
Administrative Complaint seeking fines was filed.
Florida Structures PHD, PE
Case No. 2005-048941
Probable Case was found that Florida Structures PHD, PE, of Wellington,
Florida is not licensed to practice architecture in the State of Florida and is
listed under the architect heading in the yellow pages. A Notice and Order to
Cease and Desist was issued along with the requirement that A. Ali sign an
affidavit stating the firm will refrain from offering architectural services or
words to that effect and will no longer advertise under the architect section.
If Mr. Ali fails to return the affidavit, an Administrative Complaint seeking
fines will be filed.
Goodman Design, Inc.
Earle Jay Goodman
Case No. 2005-045386
Probable Cause was found that Goodman Design, Inc. of Boca Raton, Florida is
not licensed to practice architecture or interior design in the State of Florida
and is offering interior design services on its website. The website also refers
to a Spa Building in which they claim to have provided architectural services.
Mr. Goodman appeared before the Board at the meeting which was held in February
and is attempting to qualify his firm with a licensed architect. A Notice and
Order to Cease and Desist was issued and an Administrative Complaint seeking
fines was filed.
Richard Guzman
R.G. Designs, Inc.
Case No. 2005-057926
Probable Cause was found that Richard Guzman of Bonita Springs, Florida is
not licensed to practice architecture in the State of Florida and provided
preliminary conceptual drawings for a project known as Family Medical of Naples.
Further, Mr. Guzman is offering architectural services in an advertisement and
on his webpage. A Notice and Order to Cease and Desist was issued and an
Administrative Complaint seeking fines was filed.
Charles E. Hackbarth
CH Design, Inc.
Case No. 2005-060268
Probable Cause was found that Charles Hackbarth of Port Charlotte, Florida is
not licensed to practice architecture or interior design in the State of Florida
and prepared a proposal to provide design services for a 16,000 square foot
commercial building. Mr. Hackbarth offered signed and sealed drawings from a
registered architect and his letterhead offers architectural services. Further,
Mr. Hackbarth is offering such services through a business entity without a
certificate of authorization. A Notice and Order to Cease and Desist was issued
and an Administrative Complaint seeking fines was filed.
Luis A. Lara
Case No. 2006-008074
Probable Cause was found that Luis Lara of Brandon, Florida is not licensed
to practice architecture in the State of Florida and prepared drawings for a
commercial project. Mr. Lara’s title block holds him out as an architectural
designer and an associate member of AIA. Further, Mr. Lara prepared an invoice
for architectural services. A Notice and Order to Cease and Desist was issued
and an Administrative Complaint seeking fines was filed.
Daniel Lynn
Danlyn Designs
Case No. 2005-033891
Probable Case was found that Daniel Lynn of Boca Raton, Florida is not
licensed to practice architecture in the State of Florida and is offering
architectural interiors on his business card. A Notice and Order to Cease and
Desist was issued along with the requirement that Mr. Lynn sign an affidavit
stating she will refrain from offering architectural services or words to that
effect and will change his business card. If Mr. Lynn fails to return the
affidavit, an Administrative Complaint seeking fines will be filed.
Christopher Sanchez
Case No. 2005-003446
Probable Cause was found that Christopher Sanchez of Coral Gables, Florida is
not licensed to practice architecture in the State of Florida and contracted and
provided architectural services for a residence. Mr. Sanchez also told the
complainant that he was an architect. A Notice and Order to Cease and Desist was
issued and an Administrative Complaint seeking fines was filed.
Simmons and Saray Interiors Group, Inc.
Lauren Simmons
Case No. 2005-039787
Probable Case was found that Simmons and Saray Interiors Group, Inc. of
Jupiter, Florida is not licensed to practice interior design in the State of
Florida and provided interior design plans for an office for Cline Resources.
Such services were provided through a corporation without a certificate of
authorization. A Notice and Order to Cease and Desist and an Administrative
Complaint seeking fines was filed.
Julius H. Wynn
Phoenix Construction Services, Inc.
Case Nos. 2005-057944
Probable Case was found that Julius Wynn of Tallahassee, Florida is not
licensed to practice architecture in the State of Florida and contracted and
provided architectural services for a restaurant. Further, Mr. Wynn is doing
business as Phoenix Construction Services, Inc. A Notice and Order to Cease and
Desist was issued and an Administrative Complaint seeking fines was filed.

Roger Allan Bartels
Case No. 2005-061768
Probable Cause was found that Roger Bartels of South Norwalk, Connecticut, a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Bartels when he renewed his license. Mr. Bartels was chosen for
the audit and has not provided any evidence of taking continuing education. Mr.
Bartels failed to respond to either the audit letter or this investigation.
Therefore, Mr. Bartels has failed to provide any evidence of taking any
continuing education hours during the audit period. An Administrative Complaint
seeking discipline was filed.
Christian Crookless
Case No. 2005-065592
Probable Cause was found that Christian Crookless of Garland Texas, a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Crookless when he renewed his license. Mr. Crookless failed to
respond to either the audit letter or this investigation. Therefore, Mr.
Crookless has failed to provide any evidence of taking any continuing education
hours during the audit period. An Administrative Complaint seeking discipline
was filed.
Deborah Desilets
Case No. 2005-065607
Probable Cause was found that Deborah Desilets of Miami, Florida, a licensed
architect in the State of Florida, violated Chapter 481, Florida Statutes. The
case is based on an audit of the continuing education credits claimed by Ms.
Desilets when she renewed her license. Ms. Desilets affirmed at renewal that she
completed the required hours. Ms. Desilets was chosen for the audit and has not
provided any evidence of taking continuing education. Ms. Desilets claimed
taking 20 hours of self study. An Administrative Complaint seeking discipline
was filed.
Luther L. Eubanks
Case No. 2005-056099
Probable Cause was found that Luther Eubanks of Plantation, Florida, a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Eubanks when he renewed his license. Mr. Eubanks has failed to
provide any evidence of taking any continuing education hours during the audit
period. An Administrative Complaint seeking discipline was filed.
Kevin Thomas Farrell
Case No. 2005-061834
Probable Cause was found that Kevin Farrell of Sanibel Island, Florida, a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Farrell when he renewed his license. Mr. Farrell provided
evidence of taking 20 hours that were taken outside of the audit period. An

Probable Cause was found that Gary Graham of Boston, Massachusetts, a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Graham when he renewed his license. Mr. Graham failed to respond
to either the audit letter or this investigation. Therefore, Mr. Graham has
failed to provide any evidence of taking any continuing education hours during

Janet Jarvis

Probable Cause was found that Janet Jarvis of Ketchum, Idaho, a licensed
architect in the State of Florida, violated Chapter 481, Florida Statutes. The
case is based on an audit of the continuing education credits claimed by Ms.
Jarvis when she renewed her license. Ms. Jarvis failed to respond to either the
audit letter or this investigation. Therefore, Ms. Jarvis has failed to provide
any evidence of taking any continuing education hours during the audit period.

Charles Maurice Johnson
Case No. 2005-061889
Probable Cause was found that Charles Johnson of Murfreesboro, Tennessee, a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Johnson when he renewed his license. Mr. Johnson affirmed at
renewal that he completed the required hours however, an audit showed a
deficiency. Mr. Johnson admitted that he did not complete his continuing
education hours and requested to relinquish his license. An Administrative
Complaint seeking discipline was filed.
Burton S. Landau
Case No. 2005-061902
Probable Cause was found that Burton Landau of Bloomington, New Jersey, a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Landay when he renewed his license. Mr. Landau failed to respond
to either the audit letter or this investigation. Therefore, Mr. Landau has
failed to provide any evidence of taking any continuing education hours during
the audit period. An Administrative Complaint seeking discipline was filed.
Virginia L. McGuire
Case No. 2005-021239
Probable Cause was found that Virginia McGuire of San Antonio, Texas, a
licensed interior designer in the State of Florida, violated Chapter 481,
Florida Statutes. Ms. McGuire failed to provide to the Board of Architecture and
Interior Design proof of completion of the core curriculum courses, or pass the
equivalency test of the Building Code Training Program and a citation was
issued. Ms. McGuire disputed the citation but failed to provide proof of
completion of the course and failed to pay the fine. An Administrative Complaint
seeking discipline was filed.
Timothy Charles Salyer
Case No. 2005-062094
Probable Cause was found that Timothy Salyer of Indianapolis, Indiana, a
licensed architect in the State of Florida, violated Chapter 481, Florida
Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Salyer when he renewed his license. Mr. Salyer failed to respond
to either the audit letter or this investigation. Therefore, Mr. Salyer has
failed to provide any evidence of taking any continuing education hours during

claimed by Mr. Schennum when he renewed his license. Mr. Schennnum affirmed that
he had completed the required hours. Mr. Schennum was chosen for the audit and
has only provided evidence of taking 12 continuing education hours during the

Probable Cause was found that Sondra Schiffman of Ruidoso, New Mexico, a
licensed interior designer in the State of Florida, violated Chapter 481,
Florida Statutes. Ms. Schiffman failed to provide to the Board of Architecture
and Interior Design proof of completion of the core curriculum courses, or pass

Statutes. The case is based on an audit of the continuing education credits
claimed by Mr. Shortridge when he renewed his license. Mr. Shortridge was chosen
for the audit and has only provided evidence of taking 7.5 hours and claims 12
hours for teaching at California State University. An Administrative Complaint
seeking discipline was filed.
