Case No. 2008-006724

 

Probable cause was found that A + Designs ofJacksonville, Florida, is not licensed to practice architecture in Florida and is offering these services on his title block. An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. 

 

Archit-Plans Corporation

Julian Verdeja

Case No. 2007-068845

 

Probable cause was found that Archit-Plans Corporation of Miami, Florida, is not licensed to practice architecture in Florida and is offering architectural services on a sign. A Notice and Order to Cease and Desist will be issued requiring Mr. Verdeja to sign an affidavit stating he will change his advertisement and will no longer offer architectural services.

 

April Blackwood

Case No. 2008-001482

 

Probable cause was found that April Blackwood of Jacksonville, Florida, is not licensed to practice architecture in Florida and is using the title “Interior Architect Intern” Further, Mrs. Blackwood is not in the NCARB Intern Development Program. A Notice and Order to Cease and Desist will be issued requiring Ms. Blackwood to sign an affidavit stating she changed the title and will not use the title in the future. If Ms. Blackwood fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

John Joe Crum

d/b/a The Crum Design Group

Case No. 2008-009079

 

Probable cause was found that John Joe Crum of Jacksonville, Florida, is not licensed to practice architecture in Florida and he prepared a proposal to provide signed and sealed drawings for a commercial project. Further, Mr. Crum is offering such services through a business entity without a certificate of authorization.  An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. 

 

  

Gary Cothren

d/b/a BCR Construction, Inc.

Case No. 2008-009122

 

Probable cause was found that Gary Cothren of Jacksonville, Florida, is not licensed to practice architectural services in Florida and he prepared a proposal to provide build out drawings for a commercial project. Further, Mr. Cothren is offering these services through a corporation without a certificate of authorization. An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. 

 

Designs of the Interior

Kimerlee Dodson-Prescott

Case No. 2008-001663

 

Probable cause was found that Designs of the Interior of Jacksonville, Florida, is not licensed to practice interior design in Florida . Ms. Dodson-Prescott completed a survey for the Jacksonville Business Journal which she states that the company has one licensed interior designer and offers full service interior design.  Further, the web site holds out an employee of the firm as an interior designer even though that employee is not licensed. Ms. Dodson Prescott is offering such services through a business entity without a certificate of authorization.  An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. 

 

Draft by Design, Inc.

Case No. 2008-001499

 

Probable cause was found that Draft by Design, Inc. of Boynton Beach, Florida, is not licensed to practice architecture in Florida and is offering architecture and interior design services in its brochure. A Notice to Cease and Desist offering architecture and interior design services was issued along with the requirement that Mr. Nejib sign an affidavit stating he has changed his brochure and will no longer offer such services in the future. If Mr. Nejib fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

DW Gildart, Inc.

David W. Gildart

Case No. 2008-001775

 

Probable cause was found that DW Gildart, Inc. of Orlando, Florida, is not licensed to practice architecture in Florida and is offering architecture services on its signage. A Notice to Cease and Desist offering architecture services was issued along with the requirement that Mr. Gildart sign an affidavit stating he has changed his signage and will no longer offer such services in the future. If Mr. Gildart fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

  

Form & Function Interior Design

Kathy Scott

Case No. 2007-053034

 

Probable cause was found that Form & Function Interior Design of New Smyrna Beach, Florida, is not licensed to practice interior design in Florida and is doing business as Form & Function Interior Design. A Notice to Cease and Desist offering interior design services was issued along with the requirement that Ms. Scott sign an affidavit stating she has changed her business name and will no longer offer such services in the future. If Ms. Scott fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Jill Foster

Case No. 2008-000625

 

Probable cause was found that Jill Foster of Tallahassee, Florida, is not licensed to practice interior design in Florida and is holding herself out as an interior designer in a paid newspaper article. A Notice to Cease and Desist offering interior design services was issued along with the requirement that Ms. Foster sign an affidavit stating she has changed all advertisements and will no longer offer such services.  If Ms. Foster fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Lucy Halasz

Case No. 2008-001839

 

Probable cause was found that Lucy Halasz of Venice, Florida, is not licensed to practice interior design in Florida and is holding herself out as an interior designer in the Sarasota Magazine. A Notice to Cease and Desist offering interior design services was issued along with the requirement that Ms. Halasz sign an affidavit stating she has changed her advertisement and will no longer offer such services.  If Ms. Halasz fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Mary T. Herr

Case No. 2007-048120

 

Probable cause was found that Mary T. Herr of Lake Mary, Florida, is not licensed to practice interior design in Florida and is holding herself out as an interior designer on her business card.  A Notice and Order to Cease and Desist offering interior design services was issued along with the requirement that Ms. Herr sign an affidavit stating she has changed her business card and will no longer offer such services.  If Ms. Herr fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

Case No. 2007-068978                      

 

Probable cause was found that Hirsch Bedner & Associates of Atlanta, Georgia, is not licensed to practice interior design in Florida and contracted to provide interior design services on a commercial project.  Further, Hirsch Bedner & Associates is offering these services without a certificate of authorization. An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. 

 

 

Keen Eye Home Consulting

Kerri M. Crane

Case No. 2008-008317

 

Probable cause was found that Kerri M. Crane, Keen Eye Home Consulting of The Villages, Florida, is not licensed to practice interior design in Florida and is holding herself out as an interior designer in an advertisement in The Villages Daily Sun Newspaper and on her web site.   A Notice and Order to Cease and Desist offering interior design services was issued along with the requirement that Ms. Crane sign an affidavit stating she has changed her advertisements and will not offer such services in the future.  If Ms. Crane fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Kelly Wearstler, Inc.

Kelly Wearstler

Case No. 2007-067706

 

Probable cause was found that Kelly Wearstler, Inc. of Los Angeles, California, is not licensed to practice interior design in Florida and contracted to provide interior design services on The Tides Hotel on South Beach in Miami.  Ms. Wearstler specifically offers interior design services in the contract and is offering such services through a business entity without a certificate of authorization.  An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. 

 

Key Accents

Patricia O’Connor

Case No. 2008-002512

 

Probable cause was found that Key Accents, of Key West, Florida, is not licensed to practice interior design in Florida and is holding herself out as an interior designer on her web site.  A Notice and Order to Cease and Desist offering interior design services was issued along with the requirement that Ms. O’Connor sign an affidavit stating she has changed her web site and will no longer offer such services.  If Ms. O’Connor fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Kukes Simons Interiors, Inc.

Patricia Kukes

Case No. 2007-066231

 

Probable cause was found that Kukes Simons Interiors, Inc. Boca Raton, Florida, is not licensed to practice interior design in Florida and contracted to provide interior design services through a corporation without a certificate of authorization. This is a second offense.  An Administrative Complaint seeking fines will be filed. 

 

 

 

Robert B. Lamar

Case Number 2007-057003

 

Probable cause was found that Robert B. Lamar of Pensacola, Florida, is not licensed to practice interior design or architectural services in Florida and provided architectural services and interior design services for two commercial projects.  Mr. Lamar stated he was working under a licensed Professional Engineer but the engineer was a mechanical/electrical engineer and not a civil/structural engineer.  An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. 

 

Masterworks of Pensacola, Inc.

Rod Buchanan

Case No. 2007-037739

 

Probable cause was found that Masterworks of Pensacola, Inc. of Pensacola, Florida, is not licensed to practice interior design in Florida and offered interior design services in a yellow page advertisement.  Mr. Buchanan has failed to return the affidavit as required by the Probable Cause Panel at a meeting held November 5, 2007.  An Administrative Complaint seeking fines will be filed. 

 

MDO Design, Inc.

Michael O’Connor

Case No. 2008-011121

 

Probable cause was found that MDO Design, Inc. of Ponte Vedra Beach, Florida, is not licensed to practice architecture in Florida and prepared a proposal to offer architectural services. Further, Mr. O’Connor is offering these services through a corporation without a certificate of authorization.  A Notice and Order to Cease and Desist and an Administrative Complaint seeking fines will be filed. 

 

Steven A. Mickley

Case No. 2008-003710

 

Probable cause was found that Steven A. Mickley of Jupiter, Florida, is not licensed to practice interior design in Florida and contracted to provide signed and sealed drawings for an enclosure for a small screen porch.  A Notice and Order to Cease and Desist and an Administrative Complaint seeking fines will be filed.

Probable cause was found that Moonlight Editions, Inc. of Hollywood, Florida, is offering architectural services without a certificate of authorization. A Notice to Cease and Desist was issued.

 

Neal Communities, Inc.

Probable cause was found that Neal Communities, Inc. of Bradenton, Florida is not licensed to practice interior design in Florida and is holding herself out as an interior designer in an advertisement and on the Internet.   A Notice and Order to Cease and Desist offering interior design services was issued along with the requirement that Ms. Neal sign an affidavit stating she has changed her advertisements and will not offer such services in the future.  If Ms. Neal fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Richard Opalinsky

Case No. 2007-060073

 

Probable cause was found that Richard Opalinsky of Bradenton, Florida, is not licensed to practice architecture in Florida and signed numerous documents using the title architect.  A Notice and Order to Cease and Desist and an Administrative Complaint will be filed. 

 

Properties Solution Services

ONelia M. Duran

Case No. 2007-066282

 

Probable cause was found that Properties Solution Services of Miami, Florida, is not licensed to practice architecture in Florida and offered to provide architectural services for a residential project.  Further, these services are being offered through a corporation without a certificate of authorization. This is a second offense.  A Notice and Order to Cease and Desist and an Administrative Complaint seeking fines will be filed. 

 

 

RELO Interior Services

Peter Hough

Case No. 2008-001868

 

Probable cause was found that RELO Interior Services of Tampa, Florida, is not licensed to practice interior design in Florida and is holding itself out as an interior designer in a brochure.   A Notice and Order to ease and Desist offering interior design services was issued along with the requirement that Mr. Hough sign an affidavit stating he has changed his brochure deleting all references to interior design.  If Mr. Hough fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Maxienne Samuels

Designs of the Interiors

Case No. 2008-010588

 

Probable cause was found that Maxienne Samuels, Designs of the Interiors of Jacksonville, Florida, is not licensed to practice interior design in Florida and is holding herself out as an interior designer on her web site.  A Notice and Order to Cease and Desist offering interior design services was issued along with the requirement that Ms. Samuels sign an affidavit stating she has changed her web site and will not offer such services in the future.  If Ms. Samuels fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Sherri Meadows Interiors

Probable cause was found that Sherri Meadows, Sherri Meadows Interiors of Jacksonville, Florida, is not licensed to practice interior design in Florida and is holding herself out as an interior designer in an advertisement in The Jacksonville Business Journal. A Notice and Order to Cease and Desist offering interior design services was issued along with the requirement that Ms. Crane sign an affidavit stating she has changed her advertisement and has deleted all references to interior design and space planning.  If Ms. Meadows fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

 

Trade Mark Interiors

Traci Murphy Bomberger

Case No. 2008-003875

 

Probable cause was found that Traci Murphy Bomberger, Trade Mark Interiors of Bradenton, Florida, is not licensed to practice interior design in Florida and is holding herself out as an interior designer and commercial designer on her web site.   A Notice and Order to Cease and Desist offering interior design services was issued along with the requirement that Ms. Bomberger sign an affidavit stating she has changed her web site and will not offer such services in the future.  If Ms. Bomberger fails to return the affidavit, an Administrative Complaint seeking fines will be filed.

Marshall I. Levy

Case No. 2007-046077

 

Probable cause was found that Marshall I. Levy of Fort Lauderdale, Florida, a licensed architect in Florida, allegedly misrepresented on his application the fact that he had been practicing architecture through a corporation prior to receiving his certificate of authorization. An Administrative Complaint seeking discipline will be filed.

 

Mariela Mesa

Case No. 2007-056637

 

Probable cause was found that Mariela Mesa of Miami, Florida, a licensed interior designer, failed to satisfy the terms of a Final Order that was entered on March 23, 2007.  An Administrative Complaint seeking discipline will be filed.

 

Norman D. Palmer

Case No. 2007-042296

 

Probable cause was found that Norman D. Palmer of Newberry, Florida, a licensed architect, signed and sealed drawings that an unlicensed individual prepared while his license was in a delinquent status. Mr. Palmer did not provide the required documentation that he exercised proper responsible supervisory control for plans prepared outside of his office. An Administrative Complaint seeking discipline will be filed.

 

James Michael Rinehart

Case No. 2007-007995

 

Probable cause was found that James Michael Rinehart of Tampa, Florida, a licensed interior designer, failed to produce permit and construction documents after receiving $15,000.00.  An Administrative Complaint seeking discipline will be filed.

 

 

William Edwin Wallace

Case No. 2007-065241

 

Probable cause was found that William Edwin Wallace of Jacksonville, Florida, a licensed architect, signed and sealed drawings that an unlicensed individual prepared for a commercial project without ever meeting with the client. The contract entered into by the unlicensed individual offers architectural services. An Administrative Complaint seeking discipline will be filed.
