<p>Probable cause was found that AEC Services, Inc. and Ron Fair of Tampa, FL, are not licensed to practice architecture in Florida; however Mr. Fair is a licensed Professional Engineer, and entered into a contract to provide architectural services and is using the title architect.   Further, Mr. Fair is offering these services through a corporation without a certificate of authorization. An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. </p>

<p>Probable cause was found that Hector Ampuero, Jr. of Hollywood, FL, is not licensed to practice architecture in Florida and is offering architectural services on his website.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Mr. Ampuero, Jr. sign an affidavit stating he has changed his website and will not offer these services in the future.  If Mr. Ampuero, Jr. fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

The firm’s website states the Ms. Herbert is responsible for all architectural design.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Ms. Herbert sign an affidavit stating she has changed the website and will not offer these services in the future.  If Ms. Herbert fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

<p>Probable cause was found that Make It Happen Contractors, Inc. and Lawrence Reznick of Boca Raton, FL, are not licensed to practice architecture in Florida and are offering architectural services on Craigslist.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Mr. Reznick sign an affidavit stating he has changed his advertisements and will not offer these services in the future.  If Mr. Reznick fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

<p>Probable cause was found that Michael Minton of Orlando, FL, is not licensed to practice architecture in Florida and is signing contracts over the title architect.  A Notice and Order to Cease and Desist from using the title architect was issued along with the requirement that Mr. Minton sign an affidavit stating he will no longer use the title architect.  If Mr. Minton fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

<p>Probable cause was found against Tom Mongello and Gallo Herbert Lebolo of Deerfield Beach, FL. Mr. Mongello is not licensed to practice architecture in Florida and but works for a licensed architectural firm. The firm’s website states the Mr. Mongello is a project architect.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Mr. Mongello sign an affidavit stating he has changed the website and will not offer these services in the future.  If Mr. Mongello fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

If Mr. Rimbey fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. </p>

<p>Probable cause was found that Vanishing Point Studio, Inc. and Jaime of Sunrise, FL, are not licensed to practice architecture in Florida and are offering architectural services on its website.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Mr. Scarpitta sign an affidavit stating it has changed its website and will not offer these services in the future.  If Mr. Scarpitta fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

<p>Probable cause was found that Vergel’s Construction, Inc. and Marisela Duran of Miami, FL, are not licensed to practice architecture in Florida; however Ms. Duran is a licensed General Contractor, and contracted to provide architectural services for a residence. Further, Ms. Duran is offering these services through a corporation without a certificate of authorization. An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. </p>

<p><strong>Case No. 2010-027802</strong></p>
<p><strong> </strong>Gregory John Burke and Gregory John Burke / Architect, PA, a licensed architect and architectural firm went before the Probable Cause Panel on November 1, 2010. Probable cause was found and an Administrative Complaint seeking discipline was issued. Mr. Burke requested that the case be set as reconsideration. Based on further review at the meeting held on February 16, 2011, it was determined that there was insufficient evidence to prove that a violation of Chapter 481, Florida Statutes occurred. For the aforementioned reason this case was dismissed without prejudice.</p>
<p><strong>Jaime P. Colmenares</strong></p>
<p><strong>Case Nos. 2010-012079 &amp;</strong><strong> 2010-036771</strong></p>
<p>Probable cause was found that Jaime P. Colmenares of Cape Coral, FL, a Florida licensed architect, and has failed to comply with the terms of a Final Order that was entered on April 18, 2008 by failing to provide a project list during probation. Further, Mr. Colmenares has failed to pay a fine or provide proof of continuing education pursuant to a Final Order that was entered on March 8, 2010. An Administrative Complaint seeking discipline will be filed.</p>

<p><strong>Case No. 2010-059153</strong></p>
<p>Probable cause was found that Raymond Lee Enfield of Elkhart, IN, is licensed to practice architecture in the State of Florida, and has failed to comply with the terms of a Final Order that was entered on March 8, 2010 by failing to provide proof of continuing education. An Administrative Complaint seeking discipline will be filed.</p>

<p>Probable cause was found that Jeorge Ann Samet and Interiors by Jeorge Anne of Boca Raton, FL, are licensed to practice interior design in the State of Florida, and are offering architectural services on their web site. It is also alleged that Ms. Samet actually practiced architecture on a project. An Administrative Complaint seeking discipline will be filed.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing an appearance before the Board and a fine of $1,250.00 plus costs against Gayle Barr and Country Club Designers, Inc.’s license to practice interior design.  Ms. Barr, a licensed interior designer, is offering interior design services through a corporation without a certificate of authorization.  Further, Ms. Barr was practicing on a delinquent license.</p>
<p><strong>Gerald Belgrave</strong></p>
<p><strong>Case No. 2010-020103</strong></p>
<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a reprimand and a fine of $750.00 plus costs  against Gerald Belgrave’s license to practice architecture.  Mr. Belgrave, a licensed architect, signed and sealed plans for a residential project that were prepared by an unlicensed draftsman without ever meeting with the client. Further, Mr. Belgrave has been unable to provide the proper documentation for signing and sealing drawings prepared outside of his office.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $15,000.00 plus costs against Lee Projects, LLC and Gal Levy. Lee Projects, LLC and Gal Levy, are not licensed to practice architecture and are offering architectural services on its website and appears to have done commercial work.  Further, Mr. Levy is offering these services through a corporation without a certificate of authorization.</p>
<p><strong>McHarris Planning &amp; Design</strong></p>

<p><strong>Case No. 2009-042948</strong><strong>                                                                                              </strong></p>
<p>The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $5,000.00 plus costs against McHarris Planning &amp; Design and Joseph M. McHarris. McHarris Planning &amp; Design and Joseph M. McHarris, are not licensed to practice architecture and offered architectural services in a Request for Proposal and offers commercial design services on its website. McHarris Planning &amp; Design does have a Florida licensed architect who is listed as a corporate officer; however this individual qualifies another firm and is not an employee of  McHarris Planning &amp; Design. </p>
<p><strong>James T. Melvin</strong></p>
<p><strong>Case No. 2010-012087</strong></p>
<p>The Florida Board of Architecture and Interior Design agreed to enter a final order revoking James T. Melvin’s Florida license to practice architecture.  A Final Order was entered against James T. Melvin on June 23, 2009 requiring him to pay fines and costs along with a project list for his probation period.  Mr. Melvin has failed to pay the funds and provide a project list as per specified in the Final Order.</p>

<p><strong>Case No. 2010-047471</strong></p>
<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $250.00 plus costs and complete five hours of continuing education pertaining to laws and rules against Jeorge Ann Samet and Interiors by Jeorge Anne’s license to practice interior design.  Ms. Samet, a licensed interior designer, is offering architectural services on her website and allegedly practiced architecture on a project.</p>
<p><strong>Oram H. Tonge</strong></p>
<p><strong>Case No. 2009-046868</strong></p>
<h4>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of costs, two hours of continuing education pertaining to laws and rules and two years probation against Oram H. Tonge’s license to practice architecture.  Mr. Tonge, a licensed architect, previously qualified a firm but left to operate as a sole practitioner.  Mr. Tonge continued to sign and seal drawings prepared at the firm which no longer had a licensed architect.   Further, Mr. Tonge has failed to provide documentation that he properly supervised drawings prepared outside of his office.</h4>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $2,500.00 plus costs against Raja Buchanan and Buchanan P.E. Consulting, Inc.  Mr. Buchanan and Buchanan P.E. Consulting, Inc. are not licensed to practice architecture; however Mr. Buchanan is a  licensed engineer. Mr. Buchanan prepared the architectural and interior design drawings for a commercial project. Further, Mr. Buchanan is offering these services through a corporation without a certificate of authorization.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing costs against Consultech &amp; Associates, Inc. and Robert McGoldrick. Consultech &amp; Associates, Inc. and Robert McGoldrick are not licensed to practice architecture and are offering architectural services on their website. Mr. McGoldrick was issued a Notice and Order to Cease and Desist but failed to return the affidavit.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a fine of $15,000.00 plus costs against CPS, Inc. and William E. Allison.  Mr. Allison and CPS, Inc. are not licensed to practice architecture and entered into a contract to provide architectural services for a commercial project. Further, Mr. Allison is offering these services through a corporation without a certificate of authorization.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation revoking MAH &amp; Associates, Inc.’s certificate of authorization.  MAH &amp; Associates, Inc. has an active certificate of authorization but no longer employs a licensed architect. Further, Mr. Heron continues to offer architectural services.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $10,000.00 plus costs against  Zureida Molina-Julio and Zands Design Concepts, Inc. Zureida Molina-Julio and Zands Design Concepts, Inc., are not licensed to practice architecture and contracted directly with the client to provide signed and sealed drawings for a residential project. Further, Ms. Molina-Julio is offering these services through a corporation without a certificate of authorization.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $10,500.00 plus costs against PMD Group, Inc. and Paul P. Mineo.  PMD Group, Inc. and Paul P. Mineo are not licensed to practice architecture and contracted to provide architectural services for a residence and uses the title “architect”. Further, Mr. Mineo is offering these services through a business entity without a certificate of authorization.</p>

<p><strong>Case No. 2010-016860</strong></p>
<p>The Florida Board of Architecture and Interior Design agreed to enter a final order revoking Sandra Ramos’ license to practice interior design. A final order was entered against Ms. Ramos on January 24, 2008 requiring her to pay fines and costs.</p>
<p><strong>Joel S. Schultz</strong></p>
<p><strong>J.S. Schultz Architect and Associates</strong></p>
<p><strong>Case No. 2009-060571</strong></p>
<p>The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a reprimand, a fine of $2,000.00 plus costs and two years probation against Joel S. Schultz and J. S. Schultz Architect and Associates’ license to practice architecture.  Mr. Schultz, a licensed architect, signed and sealed plans for a residential project that were prepared by an unlicensed draftsman without ever meeting with the client.  Further, Mr. Schultz has been unable to provide the proper documentation for signing and sealing drawings prepared outside of his office.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order imposing a fine of $10,250.00 plus costs against Tarlos &amp; Associates, Inc. and Chungsun Kang.  Tarlos &amp; Associates, Inc. and Chungsun Kang are not licensed to practice architecture in Florida and contracted to provide architectural services for a commercial project in Orlando. Further, Mr. Kang is offering these services through a corporation without a certificate of authorization.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order relinquishing Joy A. Vannice’s Florida license to practice interior design.  Ms. Vannice, a licensed interior designer, entered into two different residential contracts. In each contract, Ms. Vannice did not pull a building permit, hired unlicensed subcontractors, contracted to perform services outside the scope of her interior design license, and overcharged for materials. Further, Ms. Vannice is offering interior deign services through a corporation without a certificate of authorization.</p>

<p>Probable cause was found that Henri Almanzar and C-n-D Architectural, Inc. of Miami, FL, are not licensed to practice architecture in Florida. On September 22, 2008 Mr. Almanzar signed a Settlement Stipulation in relation to allegations of the unlicensed practice of architecture. On a set of drawings submitted to the Building Department in March 2009, Mr.Almanzar is doing business as C-n-D Architecture, Inc.  Further, Mr. Almanzar is offering these services through a corporation without a certificate of authorization. An Administrative Complaint seeking fines will be filed. </p>

<p><strong>Jennie K. Bedwell</strong></p>
<p><strong>Case No. 2010-047816</strong></p>
<p>Probable cause was found that ArchDWell Design + Corp and Jennie K. Bedwell of North Miami, FL, are not licensed to practice architecture in Florida and are offering architectural services on its web site.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Ms. Bedwell sign an affidavit stating she has changed her web site and will not offer these services in the future.  If Ms. Bedwell fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>
<p><strong>Roger B. Chewning</strong></p>
<p><strong>Case No. 2011-022101</strong> </p>
<p>Probable cause was found that Roger B. Chewning of Tampa, FL, is not licensed to practice architecture in Florida. Mr. Chewning is a Professional Engineer who signed and sealed plans for a residential project without the involvement of an architect. The architectural drawings were prepared by an unlicensed draftsman that entered into a contract with the client. The plans signed and sealed by Mr. Chewning include the unlicensed draftman offering architectural services. An Administrative Complaint seeking fines will be filed. </p>
<p><strong>Milton</strong><strong> Cubas</strong></p>
<p><strong>Case No. 2011-009350</strong></p>
<p>Probable cause was found that Milton Cubas of North Miami, FL, is not licensed to practice architecture in Florida. Mr. Cubas is a Professional Engineer and provided services to an unlicensed firm doing business as C-n-D Architecture, Inc. An Administrative Complaint seeking fines will be filed. </p>
<p><strong>Jose Jorge Fernandez</strong></p>
<p><strong>Case No. 2011-019455</strong></p>
<p>Probable cause was found that Jose Jorge Fernandez of Miami, FL, is not licensed to practice architecture in Florida and is offering architectural services on the Internet.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Mr. Fernandez sign an affidavit stating he has changed his advertisements and will not offer such services in the future.  If Mr. Fernandez fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>
<p><strong>Edward Lee Hall</strong></p>
<p><strong>Case No. 2010-037349</strong></p>
<p>Probable cause was found that Edward Lee Hall of Titusville, FL, is not licensed to practice architecture in Florida and contracted to provide signed and sealed drawings for a commercial project.  An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. </p>

<p>Probable cause was found that Northeast Florida Design, Inc. and Linda S. Purvis of Jacksonville, FL, are not licensed to practice architecture in Florida and are contracted to provide architectural plans for a residential project.  Further Ms. Purvis is offering these services through a corporation without a certificate of authorization.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Ms. Purvis sign an affidavit stating she has changed its documentation and will not offer these services in the future.  If Ms. Purvis fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

<p>Probable cause was found that Peter J. Shaw of Longboat Key, FL, is not licensed to practice architecture in Florida and is using the title Senior Architect in a letter.  A Notice and Order to Cease and Desist from using the title architect or words to that effect until properly licensed was issued along with the requirement that Mr. Shaw sign an affidavit stating he will no longer use the title architect.  If Mr. Shaw fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing suspension until compliance against Raymond Lee Enfield’s license to practice architecture.  Mr. Enfield failed to satisfy the terms of a Final Order that was previously entered on March 8, 2010 by failing to provide proof of continuing education.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order approving a settlement stipulation imposing a reprimand and costs against Timothy Krebs.  Mr. Krebs, a licensed architect, signed and sealed a commercial project for wind load only that was prepared by an unlicensed draftsman.  The drawings that were signed and sealed by Mr. Krebs were not of a sufficiency high standard.</p>

<p>The Florida Board of Architecture and Interior Design agreed to enter a final order relinquishing David M. Putnam’s Florida license to practice architecture.  Mr. Putnam signed and sealed plans for a residential project that were rejected three times. The homeowner was required to hire a new architect whose drawings were approved on the first submittal.  The Board’s expert consultant reviewed the file and found Mr. Putnam negligent.</p>

<p>Probable cause was found that Stiles-Sowers Construction, Inc. and Keith Sowers of Naples, FL, are not licensed to practice architecture in Florida and are offering architectural services on its web site.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Mr. Sowers sign an affidavit stating he has changed his web site and will not offer such services in the future.  If Mr. Sowers fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

<p>Probable cause was found that Frontop Digital Computer Graphic Company and Sharon Zhang of Guangzhou, China, are not licensed to practice architecture in Florida and are offering architectural services in an email solicitation.  A Notice and Order to Cease and Desist from offering architectural services was issued along with the requirement that Ms. Zhang sign an affidavit stating it has changed its advertisements and will not offer these services in the future.  If Ms. Zhang fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

<p>Probable cause was found that Brett Hertzler of Tamarac, FL, is not licensed to practice architecture in Florida and is using the title architect on his website.  A Notice and Order to Cease and Desist from using the title architect was issued along with the requirement that Mr. Hertzler sign an affidavit stating he has changed his website and will not use the title in the future until properly licensed.  If Mr. Hertzler fails to return the affidavit, an Administrative Complaint seeking fines will be filed.</p>

An Administrative Complaint seeking fines will be filed and a Notice and Order to Cease and Desist will be issued. </p>

<p>The Board entered a final order finding that Respondent was convicted of a crime, which relates to the practice of architecture.  Respondent’s license was suspended for two years. He received three years probation, a fine of $1,000.00, and the Board required additional continuing education.  Respondent was previously found guilty of mail fraud regarding alleged bribes to obtain work from the United States Post Office. </p>

<p>The Board proposed a counter settlement stipulation between the parties. The counter stipulation provides for a one-year suspension with the Board reviewing five of the Respondent’s prior commercial projects, three years probation, additional continuing education hours, and a fine of $1,000.  The complaint involved architectural services provided by the Respondent for a funeral home located in New Port Richey, Florida.</p>

Case No. 2001-09523</h4>
<p>The Florida Board of Architecture and Interior Design announces that on February 5, 2003, a final order was entered imposing a fine of $15,000.00 against Jay Leonard, Crawfordville, Florida for the unlicensed practice of architecture.</p>
<p>Mr. Leonard illegally offered and provided architectural services for the designs of the Down Under Dive Center, Ochlocknee Christian Church, and the residence of John Santagini.</p>
<p>After reviewing the evidence that showed Mr. Leonard was offering and providing architectural services, the Board entered a final order, imposing a fine of $5,000.00 for each count for a total of $15,000.00.</p>

Case No. 2001-09057</h4>
<p>The Board entered a final order finding that the Respondent lacked responsible supervisory control and sealed plans that were not of a sufficiently high standard for a project in Fernandina Beach, Florida. Respondent’s license was reprimanded, placed on two years probation, and fined $1,000.00.</p>

architectural services for the design of The Church of the Apostolic Faith. Rev.<br />
Earnest Jackson paid Mr. Lee $1,300.00 for his services, before determining that<br />
Mr. Lee was not licensed.</p>
<p>After reviewing the evidence that showed Mr. Lee was offering and providing<br />
architectural services, the Board found probable cause to file an administrative<br />
complaint seeking fines up to $5,000.00. </p>

<p>On or about October 20, 1997, Alvah Breitweiser (&quot;Respondent&quot;) and Karl<br />
Gaiser (&quot;Gaiser&quot;), entered into an agreement with Forest Oaks Lutheran Church to<br />
act as architects for a project which involved a new sanctuary and offices. On<br />
the Forest Oaks Lutheran Church project, Respondent and Gaiser did not execute a<br />
written agreement between themselves regarding each parties responsibility in<br />
violation of Rule 61G1-23.015(2), Administrative Code. Gaiser is not a licensed<br />
to practice architecture in the State of Florida. The aforementioned plans and<br />
drawings were prepared outside of Respondent&#8217;s office. The Respondent functioned<br />
as essentially a &quot;plan stamper&quot; for Gaiser&#8217;s plans since Gaiser is not licensed<br />
to practice architecture in the State of Florida. In preparation of the<br />
architectural and structural drawings contained in the Permit Drawings,<br />
Respondent was negligent in that he failed to exercise due care to conform<br />
acceptable standards of architectural practice in such a manner as to be<br />
detrimental to the public. Probable cause was found for negligence, plan<br />
stamping, and aiding and abetting unlicensed activity. An administrative<br />
complaint was filed.</p>

construction of 18 single family town homes for the sum of $86,400.00. A.D.C.<br />
Inc. is not licensed to practice architecture in the State of Florida. A.D.C.<br />
Inc. employed the services of Guillermo Gonzalez (&quot;Respondent&quot;) for the required<br />
certification of the construction drawings. Jones was unaware that A.D.C. Inc.<br />
was not licensed or that A.D.C. Inc. employed the services of Respondent for the<br />
required certification of the construction drawings. The Dade County Building<br />
Department rejected the construction documents certified by Respondent on<br />
January 23, 2001, for permit issuance. The construction documents certified by<br />
Respondent on January 23, 2001, failed to meet the title block requirements of<br />
Rule 61G1-16.004, Florida Administrative Code. Respondent did not supervise the<br />
preparation of the aforementioned plans and drawings. The aforementioned plans<br />
and drawings were prepared outside of Respondent&#8217;s office. The Respondent<br />
functioned, as essentially a &quot;plan stamper&quot; for A.D.C. Inc.&#8217;s plans since A.D.C.<br />
Inc. is not licensed to practice architecture in the State of Florida. In<br />

<p>Probable cause was found that Mr. Morris received an unsolicited gratuity of<br />
$10,000.00 from First Construction, Inc., without the knowledge or consent of<br />
his client Dennis Kessler. It&#8217;s a violation for an architect to solicit or<br />
accept gratuities from contractors for work that the architect is responsible<br />
without the knowledge of the architect&#8217;s client.</p>
<p>After reviewing the evidence that showed Mr. Morris received an unsolicited<br />
gratuity, the Board found probable cause to file an administrative complaint<br />
seeking penalties up to revocation of Mr. Morris&#8217; architectural license. </p>
